I was reading about &quot;How to configure WordPress to create search engine friendly URLs for permalinks&quot; and checking my database and noticed similar problem to that reported by Steven. 

The post_name field in wp_posts table, in some cases wrong or blank. 

I tried to change through &quot;Edit Post&quot; without success. 
But when clicked on &quot;Quick Edit&quot; and made the change it worked. 

May not be the case reported by Steven, but make an attempt. 

I have not implemented the way of the permalink in your post suggested, but will.
