Hello
I was reading about “How to configure WordPress to create search engine friendly URLs for permalinks” and checking my database and noticed similar problem to that reported by Steven.
The post_name field in wp_posts table, in some cases wrong or blank.
I tried to change through “Edit Post” without success.
But when clicked on “Quick Edit” and made the change it worked.
May not be the case reported by Steven, but make an attempt.
I have not implemented the way of the permalink in your post suggested, but will.

I haven’t faced such a problem but my friend faced it. We both worked on it to solve it but failed. I did searched to get out of this problem but couldn’t get any satisfactory response.
